Non-Medicaid Care Management

Private, non-profit organization offering professional counseling and prevention services.

For adults who do not receive Medicaid, care management is available through our Non-Medicaid Care Management program. Our care managers work with you to develop a person-centered care plan for accessing and using community services and supports that are necessary to promote your well-being.

Many people are unable to find providers and services, which makes it hard for people to get well and stay healthy. Your care manager will work with you to set up appointments and obtain the services you need to put you on the road to better health. These services and supports are provided through a network of organizations, with the care manager overseeing the process.

That network includes not only healthcare providers but other social and community supports that are essential to your overall health, such as housing and vocational services. The goal is to ensure access to appropriate services, improve health outcomes, reduce preventable hospitalizations and emergency room visits, promote the use of Health Information Technology (HIT), and avoid unnecessary care.

Services

  • Comprehensive care management
  • Care coordination and health promotion
  • Comprehensive transitional care from inpatient to other settings, including follow-up
  • Individual and family support, which includes authorized representatives
  • Referral(s) to community and social support services
  • Use of Health Information Technology to link services, as feasible and appropriate

Eligibility

  • Must be an individual 18 years of age or older with Serious Mental Illness (SMI). Includes special populations such as those receiving Assisted Outpatient Treatment (AOT) and other high-need SMI populations.
  • Must be a resident of Broome County

Cost

The Non-Medicaid Care Management program for adults with Serious Mental Illness is funded through a contract with Broome County and there is no cost to individuals referred to the program.

Referrals

All referrals for services are processed through Broome County Single Point of Access (SPOA). Upon receipt, SPOA reviews the referral for eligibility, discusses appropriateness, and facilitates distribution to an appropriate provider at the SPOA Committee meeting. Per current Guidelines, recipients are approved for twelve (12) months of Non-Medicaid Care Management at the time of initial assignment at the SPOA meeting.
