Our mobile Outreach and Engagement team provides treatment and recovery supports for individuals living in Cortland, Madison and Herkimer counties who are struggling with a substance use disorder. This multi-disciplinary team of counselors, peers and medical providers goes into the community to assess the needs of individuals and engage them with the appropriate level of treatment. Our team will also coordinate care with other providers to ensure a comprehensive continuum of care.

Peer engagement

Our Certified Recovery Peer Advocates (CRPAs) are at the center of our Outreach and Engagement Services and a point of contact for individuals and families seeking to engage with our treatment team. Peers utilize their own recovery experience and story to inspire, connect and support individuals who are struggling with addiction and in early recovery from a substance use disorder. Peers are actively working in the community through mobile/rural outreach engaging directly with people who use drugs to prevent overdose and infectious disease transmission; improve physical, mental, and social wellbeing; and offer low barrier options for accessing health care services, including substance use and mental health treatment.

Clinical services

Using screening tools, our clinicians will assess individuals in the community, provide individual, group and family counseling, and make appropriate referrals to treatment. Our skilled team is prepared to help individuals and families work through the challenges of addiction and early recovery.

Care management and treatment referrals

Our team will work with individuals to explore and address a variety of issues potentially impacted by addiction, including medical, housing, financial, employment, legal and others. Transportation to initial treatment services is available.

Telehealth

Employing emerging telepractice technologies, the team is able to bring critical treatment interventions to rural locations and remove common barriers to treatment and recovery. Our fully accessible mobile clinic allows us to deliver treatment services in the community when an individual is unable to visit one of our clinic locations.

Medication Assisted Treatment (MAT)

The Outreach and Engagement Services team is able to bring critical treatment interventions such as Medication Assisted Treatment to individuals struggling with addiction. In combination with counseling and behavioral therapies, MAT can provide a patient-centered approach to the treatment of substance use disorders.

Harm reduction

Our team will meet those who use substances “where they are at,” and address conditions of use to reduce the negative consequences of risky behaviors – including the overall harmful effects of substance use. Harm reduction kits are available upon request.
