Targeted Case Management
A short term, intensive level of support for people deemed at high risk due to health-related social needs, suicide or overdose and particularly during times of transitions.
Targeted Case Management (TCM) Services assists people receiving services in sustaining recovery and gaining access to needed medical, social, legal, educational, housing, vocational, and other services and supports.
TCM includes care coordination and supports for people deemed at high risk due to health related social needs and particularly during times of transitions such as from a residential treatment, hospital emergency department, incarceration, or psychiatric hospitalization.
