Navigator Bridget Schmidt

As part of Family & Children’s Society’s continued collaboration with the Care Compass Network and the Delivery System Reform Incentive Payments (DSRIP) project, community-based navigation services are now available to all Medicaid recipients. Navigation Coordinator Bridget Schmidt connects with individuals to determine their needs and make appropriate referrals within Family & Children’s or to other community agencies. Anyone (the client included) can refer a Medicaid recipient for navigation services.

Referrals can range from a clothing voucher, help finding a primary care provider, assistance fighting an eviction, getting a health home referral or other service. A client can be provided with services once per “DSRIP year.”
