Community Based Services
Family & Children’s Counseling Services offers a variety of community based services that are designed to:
- Help people navigate the healthcare system
- Connect people to needed services and community resources
- Advocate on their behalf with service providers and insurance companies
Each community based service is unique, with some limited to specific counties in the region. Depending on your health status and individual needs, you may qualify for assistance through one of our Community Based Services.
What exactly is Care Management?
Advocates and providers work together to make sure you get the care and services you need. Many people are unable to find providers and services, which makes it hard for people to get well and stay healthy. Community Based Services were created with these people in mind.
Depending on the specific program, services may include:
- Comprehensive care management
- Referral to health care providers including primary care, mental health and substance use disorders
- Comprehensive transitional care from inpatient to other settings, including follow-up
- Individual and family support
- Referral to community and social support services (such as food, housing, clothing, medical transportation, health and wellness programs)
- Assistance with insurance enrollment, access and prior approvals for services
